Add extra parameters to QR code

Fixes #3798

This PR adds extra properties to the geoJson represented by the LOI report QR code: the date and the survey name. There was also an adjustment to the property for the loi name, since for predefined sites this property is not present. When that happens, the geoJson will now attempt to include the id instead, if present.
