feat: replace seeded config with skip_lfs base - #43

`lhm install` no longer writes a default `~/.config/lefthook.yaml`. The one setting it needed to provide (`skip_lfs: true`) is now merged automatically as a lowest-priority base layer whenever any config layer exists, so lefthook's built-in LFS support stays suppressed and the git-lfs underlay adapter remains the sole LFS runner. Any system, user, or repo config can still override `skip_lfs`. Co-Authored-By: Claude Fable 5 <noreply@anthropic.com>
