chore(pr-template): add AI usage note

Purpose / Description

Many PRs now use AI tools. Including this note in PR templates should 1) increase adherence to the AI_POLICY among long-time contributors who may forget to make Assisted-by notes and 2) help inform new contributors about the AI_POLICY.

• Remove 'Please' - brevity/respect for time is politeness IMO

I would prefer to put the entire 'NOTE' in a comment: no action needed by default,

@david-allison Is something up with code-cov? Only the ubuntu check is failing in the merge queue, but not due to an actual test failure. Happened twice:
