docs(architecture): establish AgentOS reference stack

Summary

• add the source-controlled AgentOS reference-stack entry point and decision register
• record the accepted single-custody, A2A-transport and scoped-worker boundaries
• keep the existing AgentOS PostgreSQL task/assignment/inbox path explicitly outside the Akua remote-worker path

Status

This is the one evolving architecture Draft PR. It does not authorize implementation, migration, deletion, merge, release or production changes. Root VISION.md is deliberately unchanged pending sentence-by-sentence stakeholder review.

Validation

• git diff --check
• Markdown relative-link check
• credential-pattern scan of changed Markdown
• independent architecture review (no blocking findings; authority wording clarified before push)
