Title

docs: add stable design decision anchors

Description

Summary

  • Update the spec-driven design template to give design decisions stable IDs such as D1, D2, and D3.
  • Document that decision IDs should remain unchanged when decisions are renamed, removed, or reordered.
  • Update schema instructions to encourage relative Markdown links to design decisions, such as [D1](./design.md#d1).

Testing

  • corepack pnpm build — passed
  • corepack pnpm lint — passed
  • git diff --check — passed
  • corepack pnpm test — 4 test files failed, 180 passed, 3 skipped; 9 tests failed, 4,913 passed, 98 skipped. Several failures reported Windows symlink permission errors; the failures have not been confirmed as unrelated to this change.

Closes #2056

Summary by CodeRabbit

  • Documentation
    • Clarified that design decisions use sequential, stable IDs in headings, with titles on separate lines and documented context, choices, rationale, and alternatives.
    • Added a decision template covering these fields, along with guidance to preserve existing IDs and not reuse them.
    • Clarified that tasks connected to design decisions should link to the corresponding decision ID.

📝 Walkthrough

The spec-driven schema and design template now require stable IDs and structured details for design decisions. The task instructions require tasks that depend on or implement a decision to link to its ID in design.md.

Changes: Design decision references

Layer / File(s) Summary
Define decisions and link tasks
schemas/spec-driven/schema.yaml, schemas/spec-driven/templates/design.md
The design guidance and template specify sequential decision IDs, stable across title changes or reordering, and prompts for context, choice, rationale, and alternatives. The task guidance requires links to the corresponding decision ID in design.md.

Explanation

The changes satisfy the coding requirements in #2056. schema.yaml requires decision headings with unique IDs such as ### D1, requires the next unused ID, and states that IDs must never change or be reused after removal or reordering. It also requires tasks tied to decisions to use relative links such as [D1](./design.md#d1). design.md provides matching ID guidance and a per-decision template. The changes stay within the requested template and instruction scope.

Full details: Out of Scope Changes check

Explanation

The reviewed changes are limited to schemas/spec-driven/schema.yaml and schemas/spec-driven/templates/design.md. They add stable decision ID guidance, decision structure, and relative decision links requested by #2056. No unrelated runtime, CLI, or artifact changes are present.

Full details: Docstring Coverage

Explanation

No functions found in the changed files to evaluate docstring coverage. Skipping docstring coverage check. Docstring coverage is scoped to functions touched by this diff. Analyzed 0 functions across 0 files. (1 skipped: 1 unsupported.)

Read through this against #2056. Putting only the ID in the heading (### D1) is a nice way to settle the issue's first open question: the slug stays #d1 after a retitle, and it's still a plain clickable link in every renderer, with no HTML anchors needed.

One thing that will probably turn CI red once the workflows are approved: test/core/templates/schema-docs-instruction-parity.test.ts requires every spec-driven instruction quoted in docs-lab/reference/schemas/spec-driven/index.md to be byte-identical to schema.yaml. Since this PR changes the design and tasks instructions but not that page, I'd expect design instruction is stale in index.md (and the same for tasks). That may account for some of the 9 local failures you weren't sure about. The page also quotes the design.md template (the ## Decisions placeholder around line 299), which isn't checked by the test but will be out of date as well.

Small, optional suggestion: the issue's playground run found that risks are often the trade-offs of a specific decision, so a line in the Risks / Trade-offs guidance saying to link the decision a risk comes from ([D2](#d2)) would close that gap too.
