[BUG] Agents randomly ask permission to runs tool calls#2717

馃悰 Bug Description

When providing prompts to agents that have tools and sub agents attached, the agent, and sometimes the sub agents, will randomly require approval for tool calls, even when requireApproval is not set on any tool on any agent in the system.

馃攧 Steps To Reproduce

  1. Attach tools and sub agents with tools to an agent
  2. Run enough prompts through it

馃 Expected Behavior

No HITL is required

馃摫 Actual Behavior

HITL approval is randomly required.

  1. Using the latest LiteLLM to proxy requests to AWS Bedrock. Using sonnet-4-6 as the underlying model.
  2. All Agents are Python Declarative
