Enforcement and intent assessor improvements (ADR A.5, A.8)

Implements ADR Proposals A.5 and A.8.

DeterministicEnforcementAssessor: reprioritize hook scoring (A.5)

• Agent-specific hooks (.claude/settings.json) should score higher than traditional pre-commit hooks
• BP insight: "context file instructions are advisory; hooks are deterministic"
• Review current scoring (pre-commit 60 pts, agent hooks 30 pts) and invert or equalize
• Update docs/attributes.md

DesignIntentAssessor: check for enforcement (A.8)

• Look for hooks or lint rules that require design doc updates alongside architectural changes
• Check for hooks, skills, or AGENTS.md rules referencing design doc review
• Update docs/attributes.md

Both are about "are the guardrails actually enforced."
